End of Camp Transportation Form

Each staff member must complete this form to explain how they (and their luggage) are getting home.

Maintenance Form

Please use this form to request repairs to any facilities at Camp Barney.

Supply Request

This form is for head staff only. If you are not a member of head staff, please ask your supervisor to fill out this form for you.

Head staff, use this form to request supplies that must be purchased from outside of camp.

Logistics Request

Please use this form to request help moving large items around camp. The earlier your request is submitted, the more likely we will be able to fulfill it. There will be some times that we cannot move something exactly at the time you need it; for instance, a logistics staff might be out-of-camp for an urgent supply run or doctor’s visit. We will either move the items requested in the specified timeframe or notify the requestor that it cannot get done.

Incident Report Form

This form should be completed by a Health Center Staff member or a Head Staff member immediately following any injury/incident requiring treatment in the Health Center, any incident resulting in property damage, OR any near miss that may have otherwise resulted in significant injury or property damage.

Creative Activity Proposal Form

Unit staff should fill out this form whenever they are requesting to have a creative activity on their cabin’s schedule. This form needs to be submitted by the Wednesday Prior to Chart Sign Up and then approved in order for the creative to be put on the schedule. There is a maximum limit of 4 creatives (and 4 submissions) per Chart Sign-Up week.

Meal Out Request

Use this form to request a breakfast out or a unit meat cookout. Only one of each can be made per cabin per session. Specialty areas can also complete one of each per session.

All requests must be completed by the end of chart sign up for meals beginning on the Thursday following chart sign up.

All other meals out will be processed by Dave and given to unit heads for review.

Arts & Crafts Supply Request

Arts & Crafts supply requests are paper forms that must be completed at Arts & Crafts or the Dining Hall.
